Summary Care Record

Summary Care Record (SCR) is an electronic record that gives healthcare staff faster, easier access to essential information about you anywhere in the country, so that you can be given safe treatment during an emergency or when your GP surgery is closed.

You can opt out if you do not want your information to be used in this way. Read more about the Summary Care Record on the NHS Digital website.

Mobile Telephone Number

If you provide us with your mobile phone number we may use this to send you reminders about your appointments or other health screening information. Please let us know if you do not wish to receive reminders on your mobile.

Email Address

Where you have provided us with your email address we may use this to send you information relating to your health. If you do not wish to receive communications by email please let us know.
