Change to Online appointment requests

From 1 October 2025 you will be able to request a routine appointment online between 8am and 6.30pm. Please note these requests may not be looked at until the following day. You can request an urgent appointment between 8am and 10am. After 10am if your request is urgent you will need to ring the practice.

Patient Health Questionnaire (PHQ-9)

Use this service to submit a routine review of your depression.

You can use this service if you:

  • are registered at the surgery
  • have already seen a doctor, nurse or other healthcare professional and have been invited to do so

Before you start

We’ll ask you for:

  • your first and last name, date of birth, sex, postcode, email and phone number
  • if applicable, the details of the person you are completing the form on behalf of

Call 999 or go to A&E now if:

  • someone’s life is at risk – for example they have seriously injured themselves or taken an overdose
  • you do not feel you can keep yourself or someone else safe

A mental health emergency should be taken as seriously as a physical one. You will not be wasting anyone’s time.
