Registering with the surgery

To register with the surgery, you can either:

• use the NHS Register with a GP: Claremont Medical Centre service
• fill out a adult new patient registration (16 and over) form
• fill out a child new patient registration (under 16) form

When you register, it’s helpful to have your NHS number. You can use the NHS website to find your NHS number.
